Using account attributes in health profiles

We have the need to create health profile which relies on account attribute value rather than fix number/percentage. We might have some accounts with different goals than our standard goals for the segment. This specific goals are documented as an attribute at the account level. Did you have such a need and if so, how did you solve it?
